3 Things to Remember about Player Registration Forms
Here are three important things to remember about Player Registration forms both before and during the fall soccer season:
1. Remind your Region's coaches that they must carry a signed/eSigned copy of the Player Registration form for every player on their team at all times. In case of an emergency, this form is necessary for treatment authorization.
2. Remember that if the waiver or consent agreement on the Player Registration form have been altered in any way, the registration form is invalid. Do not accept any forms with these important areas edited.
3. If your Region is not an eSign Region, file and secure your Region's copies of the Player Registration forms and signed Concussion Information sheets in a safe place.
